Job Description
A Community-Based Navigator (CBN) will provide resources and support to clients in the areas related to addiction recovery, mental health, and life skills training and development.
The Navigator will have the responsibilities of facilitating group and transportation support within the Outpatient Facility and be responsible for engaging with clients in the community. A CBN helps patients identify and access necessary resources and recovery support.
This position reports to the Navigation Supervisor.

Founded in 2017 by Estil Wallace, we started off as a small residential treatment center and have grown to now offer 8 different addiction and mental health programs, each with customizable treatment plans.
Our clients stay with us as long as needed, with support continuing for up to a year through our navigation program. We provide thorough evaluations to ensure each person gets the care that’s right for them, setting them up for the best chance at success.

Essential Job Functions
- Work directly with clients to help actualize life skills into daily practices.
- Provide direct assistance to clients to aid in the realization and completion of recovery objectives and goals.
- Meet with clients on a regular basis to discuss their progression and struggles relative to recovery plan objectives.
- Provide direct assistance in the form of transportation to Intensive Outpatient Programming or other engagements, filling out forms, making phone calls, accompanying clients to appointments, helping them move into housing, etc.
- Assist a client with a variety of supportive interventions including, but not limited to recovery from addiction, transitioning between levels of care, dealing with mental health diagnoses, financial downturns, career change, and even family relationship issues.
- Assist in finding necessary services other than covered services to meet basic needs.
- Documentation of services into EMR System.
- Collaborating with family members or other involved parties for the purpose of maintaining or enhancing a person’s functioning and recovery.
- Outreach and follow-up of crisis contacts and missed appointments.
- Participate in staffing, treatment planning, and case conferences.
- Other responsibilities as assigned by the Navigation Supervisor/Manager.
- Travel is required for this position.
License/Certification Requirements
- Driver’s License
Required Qualifications
- The candidate who identifies as being in recovery from a substance use disorder and is willing to share details of his or her personal recovery experience. Minimum of one year of sustained sobriety.
- Certified Recovery Support Worker, or willing to obtain certification within six months of hire.
- Enthusiastic about providing support and recovery to patients in early recovery from opiate use disorders.
- Thorough understanding of stressors that can impact parents in recovery from opiate use disorders.
- Must have good computer skills such as Word, Excel, and Gmail (Email).
- Able to maintain patient confidentiality.
- Adhere to the Code of Ethics.
- Maintains current certifications for this position:
- CPR/First Aid Certification
- HIPAA Compliance Training

Pay: $18.00 per hour
